Case Managers- Lead dependency Mgr. provides continuity of care with aim of permanent placement for children through a case management model that includes developing, expanding and linking resources in the community to the needs of the children throughout the child’s experience in the system,while documenting progress.  Serves as a team leader and provides guidance to assigned staff in the delivery of case management services.  Position available Wildwood, Children’s Home Society, Mid Florida division.

 Lead Case Manager-Lead Case Manager for adoptions. Provide Leadership within unit working towards adoption of children currently in foster care.  Based in Citrus/Hernando.  Bachelor’s degree in Human Services field required along with minimum of 2 years related experience.  State of Florida Child Protection Certification required.  Position available in Tavares , Children’s Home Society.
